Disability Benefits Questionnaire (DBQ)

Our Disability Benefits Questionnaire (DBQ) Completion Service connects veterans with Board Certified Physicians who professionally complete the official VA DBQ forms available for public use. Each DBQ is prepared using your medical records and service history to ensure accurate, VA-compliant documentation that strengthens and supports your disability claim.

Overview

At Military Disability Nexus, we offer remote DBQ completion services for select conditions that can be accurately assessed via secure video consultation. Each DBQ is completed by a board-certified physician who reviews your medical records, discusses your history, and documents your condition using VA-compliant language and criteria.

A Disability Benefits Questionnaire (DBQ) is typically required or recommended when a veteran is:

Filing a new VA disability claim and needs to provide detailed medical evidence for one or more conditions
Requesting an increase in an existing disability rating and must show updated medical findings
Preparing for a VA appeal and needs to strengthen their medical documentation
Clarifying or supplementing VA records when prior C&P exams were incomplete or inaccurate

We do not complete DBQs that require in-person physical exams or specialized diagnostic testing. This ensures every report we provide is accurate, ethical, and within medical best-practice standards.
